Physician or Physician Assistant - Sports Medicine

Great Lakes Orthopedics & Sports Medicine, PC (GLO ) is a multi-specialty group with three locations in St. John, Crown Point and Lowell Indiana. Great Lakes Orthopedics & Sports Medicine, PC offers patients compassion, innovation, and quality care throughout Northwest Indiana and beyond. Great Lakes Orthopedics & Sports Medicine, PC is seeking a full-time Sports Medicine Physician or Sports Medicine Physician Assistant. You will work independently, along with an outstanding, experienced support staff. You will be in clinic Monday thru Friday.

You will diagnose conditions, create treatment plans, and prescribe medications as related to the treatment plan. Responsibilities include:

• Formulate a diagnosis from assessments conducted within the clinic setting.
• Examine and diagnose patients and develops treatment plans.
• Document treatment plans in the EMR.
• Coordinate care for patients.
• Perform inpatient and outpatient consults as needed
• Provide comprehensive and continuing medical care for patients.

· Initiate requests for laboratory tests and x-rays, as appropriate.

· Prepare and document appropriate history, physical examinations and diagnosis.

· Initiate requests for tests and imaging as appropriate.

· Prescribe medications, as appropriate.

Requirements

To be considered for this position, applicants need to meet the qualifications listed in this posting.

Required Qualifications:

• Indiana license.
• BC/BE fellowship trained in Sports Medicine for the physician position. 
• Basic Life Support, certified by American Heart Association
• 2+ years relevant clinical experience preferred; will consider recent graduates.
• Ability to communicate with patients, primary care team members and management staff
• Ability to make care decisions based on best medical practices without direct supervision or guidance by a physician
• Basic technical proficiency in the use of computer programs for medical records or internet search for clinical practice
• Advanced assessment and treatment plan implementation skills
• Ability to complete clinical care lifting in a provider office environment
