Required: strong ambulatory epic in basket skills, phone triage and primary care or ambulatory clinic care coordinator experience. Wellness and prevention care Treatment for minor injuries and common illnesses Management of chronic conditions, like diabetes and high blood pressure Behavioral care Family Medicine at East Portland clinic is a primary care Family Medicine clinic and Federally Qualified Health Center (FQHC). The patient population includes all ages of the life spectrum including pregnant people, babies, children, adolescent, adults, and geriatric. The patients are generally moderately stable to moderately unstable, with or without complex acute or chronic illnesses and diseases, and SDOH barriers that require medical and nursing interventions. This role will focus primarily on telephone triage and advice, while also responding to patients urgent needs in clinic or telephonically. Flexibility and a nimble attitude are key.
