The MDS Coordinator – RN is responsible for the oversight, coordination, and completion of the Minimum Data Set (MDS) assessments and related care planning processes. This role ensures timely and accurate submission of assessments in compliance with federal and state regulations and promotes quality outcomes through coordination with the interdisciplinary team (IDT).

Duties and Responsibilities

Coordinate and oversee MDS assessments using appropriate forms based on the assessment type.
Participate in the monthly Triple Check process to validate billing accuracy and Medicare compliance.
Ensure appropriate and accurate completion of the physician certification and recertification form, Skilled Nursing Facility Advance Beneficiary Notice of Non-Coverage (SNF ABN) and Medicare non-coverage letter.
Monitor resident condition changes and collaborate with the IDT to ensure assessments reflect clinical status.
Transmit MDS to the Centers for Medicare and Medicaid Services (CMS) information system for each resident contained in the MDS in a format that conforms to current formatting standards within the prescribed time frames.
Participate in facility surveys (inspections) made by authorized government agencies
Serve on, participate in and attend various other committees of the facility (e.g., policy advisory, QAPI, etc.) as required and as appointed by the Administrator.
Assist the nursing staff in encouraging the resident and his/her family to participate in the development of a person-centered plan of care; review and revise the plan of care as required.
Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
Ensure that a comprehensive person-centered care plan is developed within 7 days after completion of the comprehensive assessment.
Conduct training programs for appropriate staff on the completion of the MDS and use of the RAI Manual.
Works beyond normal working hours and on weekends and holidays when necessary.
Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.
Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents' needs.

Qualifications

Education:

Graduate of an accredited nursing program; must possess a current RN license in the state.
RAC-CT certification is preferred

Experience:

Minimum of two (2) years’ experience as a nurse supervisor in long-term care, hospital, or similar healthcare setting.

Specific Requirements:

Must be able to read, write, speak, and understand the English language needs its own bullet
Must possess a current unencumbered active license to practice as a Registered Nurse (RN) in the state. Must be able to read, write, speak and understand the English language.
Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies, procedures, etc., that are necessary for providing quality care.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public

Physical & Sensory Requirements:

Must be able to move intermittently throughout the workday.
Must be able to speak and write the English language in an understandable manner.
Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move and/or carry such weight a minimum distance of 50 feet.
