Newport Meadows Nursing and Rahabilitation Center is searching for an experienced Social Service Assistant to join our Long-Term Care team! Long term care experience with the geriatric population is required for this position. Job Type: Part Time, 4 days a week, can be flexible About Us We are a 139-bed facility offering long term care and short-term stay. Social Service Assistant - Job Responsibilities: Orients resident and family to nursing center, helping both adjust to new environment, and offers support as needed Completes social history and MDS assessments, and develops care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken Conducts/participates in resident care conferences with resident, family, and appropriate staff Advocate and promote resident rights Receives complaints from residents and families, forwarding complaints to appropriate staff to ensure resolution. Documents complaints. Follows facility policies and procedures Performs other related duties, as assigned Education & Qualifications Bachelor's degree from accredited school in Social Work or related field preferred. 1 year experience in long-term care is required A physical examination and/or evidence from a physician indicating freedom of communicable disease to be available upon employment. Must be in good physical and mental health. Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care Must possess the ability to deal tactfully with personnel, resident, family members, visitors, government agencies/personnel, and the general public. Knowledge of elderly, ill and/or disabled. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Ability to work independently or part of a group. Effectively communicate with others.
