Worldshine Adult Medical Day Care, located in Frederick, MD is currently seeking an energetic team player to join our growing team as a Social Work Assistant

Responsibilities of this key position include:

• Develop social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken.
• Be actively involved in pre-admission and admissions processes.
• Ability to listen and act compassionately with participants/family members; must be able to work on own and maintain job related jobs
• Facilitates the social and psychological well-being of participants and their families.
• Effectively communicates needs, concerns, or problems, both written and verbal, to improve the quality of participant care.
• Conduct assessments on residents through diverse methods such as questionnaires, direct interviews, and observation.
• Assist with the development of care plans
• Assist with provide case management support
• Work as part of marketing team and participate some of marketing events

Qualifications: 

• Excellent communication skills both written and verbal.
  
• Strong documentation skills
  
• Previous Experience as aSocial Services Assistant preferred
• Prefer knowledge of Long Term Care Facility
